Leapfrog Group Hospital Safety Grade state rankings looked at nearly 3,000 hospitals across the U.S to compile the list. A letter grade is issued to each state representing overall performance in keeping patients safe from medical errors and preventable harm. The list showed of 72 hospitals graded in Virginia, 40 of them, or more than 56-percent, received an "A" grade. It puts the Commonwealth second in the nation behind only Utah.
